A team led by Dr Shraddha Deshpande, Consultant Plastic Surgeon, Wockhardt Hospitals, Mira Road, effectively saved the legs of a 33-year-old woman following a bus accident in which both of her legs suffered severe degloving injuries from being run over by the bus wheel.


33-year-old woman Kinjal Shah, an advertising executive by profession was travelling back home after a hectic day at the office.  Disaster struck in the worst possible way as when she was boarding a crowded bus, her hand slipped from the handlebar and she suddenly found herself underneath the bus wheel. The driver promptly stopped the bus, and her fellow passengers quickly pulled her out. Unfortunately, the bus wheel had already run over both her legs, for which she was immediately shifted to Wockhardt Hospitals, Mira Road. The patient was admitted to the ICU. After stabilising her, an MRI was done, which showed severe degloving injury to both her lower limbs. The skin and muscles over her thighs and legs had been completely crushed due to the shearing force and there was severe blood loss of more than 800 ml.


Dr Shraddha Deshpande, Consultant Plastic Surgeon, Wockhardt Hospitals, Mira Road added, “The patient was taken in for surgery immediately.  After being taken in for surgery, the necrotic tissue was removed and specialised negative pressure therapy was administered. Multiple blood transfusions and higher antibiotics were given. The intensive care team managed her effectively while she underwent 3 surgeries to reconstruct both lower limbs.  Due to the prompt action, the patient recovered completely and was discharged within a month of her accident.

It's crucial to identify and treat degloving injuries promptly as they can be extremely dangerous.  Degloving injuries are usually due to blunt shearing force and are closed injuries. The blood loss is hidden and the tissue damage in such cases is much more than what appears from outside. Such injuries are rare and if not identified in time can lead to severe infection, and sepsis and can be life-threatening. Utilising advanced wound care and promptly reconstructing the wounds can result in faster recovery, improved functionality, and better outcomes.
